Planning ahead

I am on a mission to help more people plan ahead for their care journey. 


We tend to approach care providers when we are at our most vulnerable; when we are unwell, lonely or in urgent need of help. This last minute rush puts us at greater risk of not getting the right quality of care. Planning ahead, even just a little bit, empowers you to interview the right providers, provide them with your plan on how to you want to be cared for, and helps you and loved ones to manage the costs more effectively.

Case Management

For over 20 years, I have managed the care delivery for many hundreds of people, at some of the UK's leading care providers. I understand your rights and will always champion for high quality, compassionate care. I know the expectations and regulations of care companies and the barriers that can be overcome to receiving more safe, caring, effective, responsive and well-led care.

Most Care providers want to do their absolute best for you. The challenges they face are real and growing. However, many do not truly have effective systems and procedures in place for investigating or resolving your concerns. Some are experiencing compassion fatigue and  could benefit from our help.

What is "Case Management"?

Independent Care Oversight and Family Representative Service

Many families assume a care home, social worker or home care agency is keeping an eye on everything. 


The reality is that no one is coordinating the whole picture. That's where independent case management can make all the difference. A case manager steps in to do the following:

 

  • Regular welfare visits 
  • Attendance at care reviews 
  • Liaison with providers 
  • Monitoring care quality 
  • Escalating concerns 
  • Hospital support 
  • Family updates

"I live 300 miles away from Mum. How do I know she's receiving good care?"

We will be your eyes and ears.  We'll connect all of the right people and make sure the care delivered is as it should be. You'll receive regular updates from us, giving you peace of mind that your loved one is happy and safe.

"What happens when a hospital discharge goes wrong?"

This is much less likely to happen with a case manager. 


Unsafe hospital discharges happen all of the time unfortunately. This is often a result of poor communication between services, or simply not having someone around to think of the small things. Perhaps no one has checked if the home is ready and suitable, or that arrangements are in place to continue care. We've been there and seen it. People discharged without electric meters topped up, no walking aids, food in cupboards or medication delivered.

We have read hundreds of care home CQC inspection reports. We've seen the trends. Too few staff, no meaningful interactions and activities. Reviews completed without family involvement. We've visited homes and seen unreported wounds, falls and medication errors. 


A good case manager will not let this happen.

The process for engaging Caremor Consultancy typically begins with an email or telephone call to book an initial consultation. This will help us understand your needs and goals. After the consultation, we'll send you a service agreement plan that will explain more.


We believe that effective change is most often possible with your existing provider. It is rare that changing care provider is the best course of action. Communicating clearly and frequently, and getting the right support from us will most often achieve a happier outcome for everyone. 


Yes, absolutely. In fact, this is exactly why some people choose to have our support. Living overseas and managing a loved one's care is really challenging. We're here to help and we're happy to work around your time zone to speak when you need us. Likewise, we're happy to support even if you live in the same home!


Yes. We have experience of managing complex and clinical care services, including spinal and brain injuries, dementia and much more. We also have connections with excellent and experienced professionals to seek advice and support from. 


In most circumstances we will ask you to sign a privacy and data protection agreement so that we may speak on your behalf or simply store information necessary to support you. Most of our services don't require any contract. Our case management services do require a contract but can be cancelled with as little as 48-hours notice, with no cancellation fees.  


Most of our services are payable by direct debit or bank transfer within 7 days of receiving your invoice. For more information, please refer to your service agreement plan that will be provided to you after the consultation.

Caremor Consultancy is aware of its obligations under Data Protection Regulations and is committed to processing your data securely and transparently. This privacy policy explains how, in line with regulations, the types of data that we may hold for you as a customer. This policy explains how we use that information, how long we keep it for and other relevant information about your data.

Data protection principles


In relation to your personal data, Caremor Consultancy will:

  • process it fairly, lawfully and in a clear, transparent way
  • collect your data only for reasons that we find appropriate in order to deliver your service and in      ways that have been explained to you
  • ensure it is correct and up to date
  • keep your data for only as long as we need it 
  • not use it for anything that you are not aware of or have consented to


Types of data we process


We hold many types of data about you, including:

  • your personal details including your name, address, date of birth, email address, phone numbers
  • your photograph
  • gender 
  • marital status
  • dependants, next of kin and their contact numbers 
  • medical or health information including any mental or physical health conditions
  • medication, you are taking and any help you need to take it
  • dietary requirements
  • assistance you require with personal care and daily activities
  • your hobbies and social activities
  • details of accidents and incidents and actions taken to help you manage any significant risks you face
  • issues relating to your general welfare
  • how you prefer to communicate
  • any safeguarding concerns and actions that have been taken as a result
  • reviews and plans relating to your care needs and the services we provide as a result of these


How we collect your data


We collect data about you in a variety of ways and this will usually start when we receive an enquiry from you or your representative(s). This includes information you and/or your representative(s) have provided to us in order to secure our services. In some cases, we will collect data about you from third parties, such as your care provider, GP, Physio Therapists or District Nurses for example. 

Personal data is kept in secure files and IT systems.


Why we process your data


The law on data protection allows us to process your data for certain reasons only:

· in order to perform the service agreement that we are party to

· in order to carry out legally required duties

· in order for us to carry out our legitimate interests

· to protect your interests and 

· where something is done in the public interest.


All of the processing carried out by Caremor Consultancy falls into one of the permitted reasons. Generally, we will rely on the first three reasons set out above to process your data. For example, we need to collect your personal data in order to carry out the service agreement that we have entered into with you and ensure you are safe and well supported


We also collect data so that we can carry out activities which are in the legitimate interests of the Company. We have set these out below:


· making decisions about who we can legally consult with

· consulting you on making decisions about when services might need to change due to a changes in your condition or forces outside of our control

· to invoice you for services provided

· maintaining comprehensive up to date care records about you to ensure, amongst other things, effective correspondence can be achieved and appropriate contact points in the event of an emergency are maintained

· effectively monitoring both your health and wellbeing when providing case management services

· offering a method of recourse for you regarding any complaints you or your representative makes on your behalf

· gaining professional opinion when making decisions about your health and wellbeing 

· dealing with legal claims made against us

· preventing fraud

· ensuring our administrative and IT systems are secure and robust against unauthorised access


Special categories of data

Special categories of data are data relating to your: 


· health

· sex life

· sexual orientation

· race

· ethnic origin

· political opinion

· religion

· trade union membership 

· genetic and biometric data.


We must process special categories of data in accordance with more stringent guidelines. Most commonly, we will process special categories of data when the following applies:


· you have given explicit consent to the processing 

· we must process the data in order to carry out our legal obligations 

· we must process data for reasons of substantial public interest

· you have already made the data public. 


We will use your special category data:


· to ensure the services we provide to you are adequate to meet your needs

· to ensure we have the relevant information to adapt/change the services we provide to you to meet any changes in your needs

· to protect you against abuse or harm

· to protect ourselves against abuse or harm


If you do not provide your data to us


One of the reasons for processing your data is to allow us to carry out our duties in line with your service agreement. If you do not provide us with the data needed to do this, we will unable to perform those duties e.g. meeting your care needs. We may also be prevented from confirming, or continuing with, your service in relation to our legal obligations if you do not provide us with this information e.g. any medical, mental or physical health issues 


Sharing your data


Your data will be shared with your chosen representatives and third parties where it is necessary for them to undertake their duties. This includes, for example, carer providers assisting you or family members acting in your best interest or by legal authority. 


We share your data with third parties in order to ensure we are meeting our legal and contractual obligations. We do not share your data with bodies outside of the European Economic Area.


Protecting your data


We are aware of the requirement to ensure your data is protected against accidental loss or disclosure, destruction and abuse. We have implemented processes to guard against such occurrences. 

Where we share your data with third parties, we provide written instructions to them to ensure that your data are held securely and in line with data protection regulation requirements. Third parties must implement appropriate technical and organisational measures to ensure the security of your data.


How long we keep your data for


In line with data protection principles, we only keep your data for as long as we need it for, which will be at least for the duration of service agreement, though in some cases we will keep your data for a period after your service contract has ended. Retention periods can vary depending on why we need your data for further information on retention periods, please contact Dean Morley at dean@caremorconsultancy.co.uk


Automated decision making


No decision will be made about you solely on the basis of automated decision making (where a decision is taken about you using an electronic system without human involvement) which has a significant impact on you.


Your rights in relation to your data


The law on data protection gives you certain rights in relation to the data we hold on you. These are:


· the right to be informed. This means that we must tell you how we use your data, and this is the purpose of this privacy notice

· the right of access. You have the right to access the data that we hold on you. To do so, you should make a subject access request. 

· the right for any inaccuracies to be corrected. If any data that we hold about you is incomplete or inaccurate, you are able to require us to correct it 

· the right to have information deleted. If you would like us to stop processing your data, you have the right to ask us to delete it from our systems where you believe there is no reason for us to continue processing it

· the right to restrict the processing of the data. For example, if you believe the data we hold is incorrect, we will stop processing the data (whilst still holding it) until we have ensured that the data is correct 

· the right to portability. You may transfer the data that we hold on you for your own purposes

· the right to object to the inclusion of any information. You have the right to object to the way we use your data where we are using it for our legitimate interests

· the right to regulate any automated decision-making and profiling of personal data. You have a right not to be subject to automated decision making in way that adversely affects your legal rights. 


Where you have provided consent to our use of your data, you also have the unrestricted right to withdraw that consent at any time. Withdrawing your consent means that we will stop processing the data that you had previously given us consent to use. There will be no consequences for withdrawing your consent. However, in some cases, we may continue to use the data where so permitted by having a legitimate reason for doing so.

Making a complaint

The supervisory authority in the UK for data protection matters is the Information Commissioner (ICO). If you think your data protection rights have been breached in any way by us, you are able to make a complaint to the ICO.
