Having a well written, person-centred care plan that is tailored to you and your needs is essential to receiving high quality, safe care. This document provides instructions to your network of health and social care professionals on how to best care for you. The standard of care plans and risk assessing can vary greatly, depending on the provider and the individual writing the plan. Our plans are comprehensive and will help set the right expectations with your care provider.
We will discuss your existing care delivery with you, review your care plan and recommend services, people and products that could benefit you. We'll share a comprehensive report with recommended actions that you can share with your care provider. We can act on your behalf if you wish and it may be worth looking at our case management services.
It is not always possible or convenient for loved ones to completely oversee and manage your care delivery. At Caremor Consultancy we can act as a case manager to oversee your service delivery, be a point of contact and make sure that you receive safe, caring, responsive, effective and well led services. Decisions will always be made in accordance with the Mental Capacity Act and other legal frameworks.
We are very conscious that the costs of care are already high. That's why we have kept our rates to an absolute minimum. Helping you is our most important goal and we care more about getting the best outcomes, rather than making huge profits.

On-site initial consultations are charged at our hourly rate.
We are happy to be as flexible as possible to support you. Please always ask if you need extra support on weekends and bank holidays. You will not be charged extra but availability may be limited.
